Our PONY Medical Release form is currently available for you and your use on the PONY Website.  It is available under “General Information & Standard Forms” as well as under the Baseball Roster Links under our “Baseball” heading.
 
New this year the PONY Medical Release Form is also available in Spanish.  Links for both the “English” and “Spanish” versions are available where the form is downloadable.
 
Some confusion has arisen on what needs entered into the “Insurance Company” and “Policy of Certificate Number” fields on the form.  These fields are for the parent’s medical insurance information that covers their child.  It is not for the league’s/team’s insurance information.
 
Please note that this is a required form for all players participating in PONY events, and must be kept by the team’s manager with his teams information and roster.  Players without the medical release form should not be able to participate until a form is filled out and available for legal indemnification of PONY Baseball and you as a PONY Field Director at the event.
